Promotion Period: 1st May – 31st August 2010

1.To qualify for your 3 Blu-ray™ discs, you must purchase one of the following Panasonic Blu-ray products: DMP-BD65EB, DM P-BD85EB, DMP-B500, DMP-B100, DMR -BS750EB, DMR-BS850EB, DMR -BW780EB, DMR -BW880EB, SC-BT230EB, SC-BT735EB, SC -BT330EB, SC -BT222EB, SC -BTX75EB and SC-BTX77EB from an authorised Panasonic dealer, between 1st May – 31st August 2010.
2. T o redeem your 3 gift copy Blu-ray™ discs, please complete your details opposite and send this claim form with a copy of your till receipt and a stamped address envelope with postage affixed to the value of £2.24 for the UK and €3.41 for ROI to the following address: Blu-ray Film Promotion, Panasonic UK, PO Box 716, High Wycombe, HP11 1XT.
3. Purchase of DM P-BD45EB from an authorised Panasonic dealer between 1st May and 31st August will entitle you to a gift copy of Avatar DVD Blu-ray only.
4. To redeem your gift copy of Avatar Blu-ray DVD, please complete your details below and send this claim form with a copy of your till receipt and a stamped address envelope with postage affixed to the value of £1.72 for the UK and €2.15 for ROI to the following address: Blu-ray Film Promotion, Panasonic UK, PO Box 716, High Wycombe, HP11 1XT.
5. D iscs will be sent to UK and ROI addresses only.
6. Blu-ray™ discs are only available while stocks last and no responsibility can be taken should disks not be available at the time of your claim.
7. T he Blu-ray™ titles are as stated and no alternative gifts are available.
8. D iscs will be despatched within 28 days of receipt of your application, however please note that no discs will be despatched prior to 28th May 2010 owing to the release date of Minority Report.
9. I f you have not received your discs 45 days after posting, please contact the administration team on Tel 01344 706723 or email customer.care@ panasonic.co.uk (with “Blu-Ray Promotion” as the subject).
10. N o responsibility can be taken for claims lost, delayed or damaged in the post.
11. T he closing date for all claims is 30th September 2010.
12. Panasonic UK Ltd reserves the right to withdraw or amend the promotion as necessary due to circumstances outside its reasonable control.
13. A ny personal information supplied by you will be used by Panasonic solely for the purpose of administering this promotion.
